How to Access HF/HTN Reports From the Dashboard

HF/HTN Reports are found in a separate tab from Rhythm reports in Rhythm360. By default, the Dashboard and Clinician Review pages display Rhythm reports when you first open the Rhythm360 portal.


Types of HF/HTN Reports

HF/HTN Reports are categorized by their Session Triggers. There are several trigger types with different meanings. 

  • 30 Days Report/Technical Report+Professional Report: Related to the report generated every 30 days from the date the patient was enrolled into the program.
  • Monthly Report/Professional Report: Related to treatment management services and related time spent.
  • Alert: This is caused by the patient's measurements being outside of the thresholds set for a period of time. To learn more about HF/HTN Alerts, please refer to the HF/HTN Alerts article linked at the bottom of this article. 

Accessing HF/HTN Reports

  1. Navigate to the Dashboard or the Clinician Review page in Rhythm360.
  2. At the top, within the alerts section, click on the tab titled HF/HTN Reports.
  3. The Dashboard will switch to HF/HTN view, displaying all HF/HTN Reports, this will be a mix of 30 Day Reports, Monthly Reports, Onboarding Checklists, and Alerts.
