Adding a New Rhythm Patient

This feature is currently only available for clinics with version 35.4.3 which can be found by selecting your icon drop down in the upper right corner of your portal. 

When a Rhythm patient transmits their first remote transmission to the Rhythm360 portal, a profile is automatically created for them. However, you may need to occasionally create a patient profile manually. Follow these steps to manually add a patient profile to Rhythm360:

  1. Login to the Rhythm360 portal for the patient's clinic.
  2. Click Add New and select Patient.
  3. A new window will appear, displaying a form for creating a new patient profile. Fill out the form using the patient's information, the required fields have a red * (only Patient Name and Date of Birth)

4. Once all of the required information has been filled out, click Continue. This will create the patient and open a window for devices to be added. 

5. Select the type of device with Data Type, in this case, a Rhythm Device and more fields to fill out will appear. 

6. Select the patient's Device Type, Manufacturer, Model, enter in the Date of Implant, Reason for Implant, and Serial Number. Once you have confirmed the information is correct, click Add New Device. This will add the device to the right hand side and other devices can be added as needed. 

7. A new window will appear, asking if you would like to set up a report schedule. If "Yes, Setup" is selected, you will have options to schedule either Heart Failure checks, Device Checks, or both. You can also setup a schedule by navigating to "Manage Devices" and selecting "Report Scheduling" under the respective device name.

8. If you are unable to add the device information at this time, this page can be accessed from the Manage Devices section of the patient page. Follow steps 5 & 6 to add the device at that time. 

If you are adding a Rhythm device for an existing patient, then steps 4&5 can be reaccessed by choosing the Manage Device page.
